Linkages, a program within Jovenes de Antaño Senior Services, was established in the Summer of 1999. It's "primary goal is to prevent or delay the premature or inappropriate institutionalization of frail older persons and adults eighteen (18) yeasr of age or older who suffer from disabilites." |
In addition, Linkages sites, located across California, are responsible for coordinating with local home and community-based service providers and carrying out a proactive role in Long-Term Care system development for their local communities. With the signing of a 1998 Governor's Initiative, there are 53 Linkages sites throughout the 33 Area Agencies on Aging. |
How is the program conducted? |
Linkages provides for comprehensive case management, along with information about and ongoing assistance in accessing community services. The Linkages Program also assumes responsibility for promoting coordination and cooperation among service providers in order to increase understanding of their respective roles in enhancing local systems of service delivery. |

Eligibility Requirements |
• You are a resident with San Benito County; • You are eighteen (18) years or older; • You have difficulty performing daily activities, such as money management, eating, bathing, taking your medications, and dressing; • You have difficulty managing your affairs due to emotional or cognitive impairment; • You are otherwise impaired by virtue of a significant event or circumstance that has occurred within the last 12 months; • You are at-risk of entering a skilled nursing facility without the provision of one or more Linkages Program components; • You have a need for Case Management and are willing to participate in Linkages; • You are capable of living independently in the community through the use of Linkages Services; |
What services does the Linkages program provide? |
• Information about services available in the community and assistance in obtaining them. • Assessment and Planning by coordinating home-based services, individualized plans, and service arrangements. • Monitoring through regular contact with the participants to make sure that services are being recieved as planned. |
